If you are a new patient to our office, the attached file contains our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival.  Thank you and please call our office if you have any questions at all.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. When more extensive dental care is necessary, financial arrangements can be made with our office.

About Our Fees

We base our fees on our knowledge, expertise, and service.  You'll find them consistent with the quality dental care we provide you and fair.

We will clearly list our fees on an itemized form.  If you don't understand something, please ask us about it.  We would be very pleased to review it with you.

Dental Insurance

As a service to our patients, our practice accepts most dental insurance programs, including non-managed care, indemnity (traditional) and PPO out-of-network.  We are not part of any managed care network.  Our accounting staff will prepare all the necessary forms for your dental benefits. However, we remind you that your specific policy is an agreement between you and your insurance company. Please keep in mind that you are responsible for your total obligation should your insurance benefits result in less coverage than anticipated. Our staff will gladly submit a pre-treatment estimate to your insurance company so that you will know what your benefits will be.

The fees charged for services rendered to those who are insured are the usual and customary fees charged to all our patients for similar services. Your policy may base its allowances on a fixed fee schedule, which may or may not coincide with our usual fees. You should be aware that different insurance companies vary greatly in the types of coverage available. Also, some companies take care of claims promptly while others delay payment for several months.

We are also work with CareCredit, an independent "patient payment plan provider" that allows you to borrow money at no interest for one year and make monthly payments for any dental care you may need.

Dentistry Today

We provide state-of-the-art dentistry and deliver your care with less stress and very little discomfort. 

Our staff is trained to help you relax.  We'll discuss procedures with you ahead of time and give you tips on getting ready for your appointment.  We also have a flat panel media entertainment center overhead for you to watch while you are here for your appointment.  You may watch any of the various channels on Cable TV or bring your own VHS or DVD.  We have found that having the distraction of the screen makes your appointments with us very entertaining and enjoyable.  Our office also offers the choice of using Nitrous Oxide to relieve anxiety.

Your First Visit

We are very excited to meet you and make your first visit as comfortable as possible.  Please arrive 15 minutes early to fill our your patient registration and medical history information.  Plan on about 90 minutes for your first visit with us so we can thoroughly review your dental conditions and needs.  Let us know if you need to take medication before we work on your teeth.  Also, please bring along any medications you currently take so we can record types and dosages.  The more we know, the better we can care for you.

We'll also need information about your insurance coverage.  Let us know if anything changes between visits so we can update your records.


We'd like to see you every six months to keep your teeth in top condition.  NO matter how well you brush and floss, we must professionally clean your teeth to remove plaque buildup.  We can also spot the early stages of gum disease and start treatment before you have serious problems.

It's easy to forget about regular dental check-ups, but they're exactly what you need for good dental health.

So, for your health and convenience, we'll automatically set up appointments for you every six months.  We want to keep you smiling!


We are now taking all digital x-rays in our office. They offer the advantage of a 90% reduction in radiation, and allow us to alter brightness and contrast as well as use color contrast in the image.  This ability to enhance the images has enabled us to better diagnose conditions before they become problems.  Besides detecting cavities, x-rays help us uncover cysts, abscesses, tumors and bone loss, also known as gum disease.  They also help us spot malformed or impacted teeth.   

Dr. Primrose has also invested in a 3-D machine that allows him to have the most diagnostic information to provide you with the greatest possible care.


Regular checkups and cleanings benefit you by:

  • Protecting your natural teeth.
  • Keeping your smile bright and your mouth's appearance the best it can be.
  • Saving future discomfort, possible loss of teeth, and unnecessary dental bills by identifying and treating small problems before they become big ones.
  • Show you care by brushing and flossing daily, eating well-balanced meals, and visiting us regularly.
